Criteria for Aquatics Program:

 

AQUATIC FUNDING PROGRAM – Guidelines & Procedures

The Special Needs Foundation of Des Moines County grants financial assistance to individuals who physically and financially qualify for our Aquatic Funding Program. To apply for Aquatic Funding: 

Applicant, please comply with these guidelines and procedures: 
1. Resident of Des Moines County, Iowa 

2. Complete the Aquatic Funding Application

3. Physician referral form signed by your doctor stating your diagnosis and a health history form (please contact your physician to go to our website and find their "Aquatics Physician Form" to fill out and submit to us)

4. Documentation from the Department of Human Services such as medical or food assistance or FIP qualification. 

-OR- 
If an applicant does not receive services from DHS but still qualifies based on income (see chart below), the applicant must submit the most recent tax form with completed application. 


6. Once completed, the Special Needs Foundation of Des Moines County Aquatic Committee will review the application. When the application is approved or denied, notice will be sent by mail. 

7. If approved, please submit the approval letter to SEIR Medical Health and Fitness to complete their enrollment process and begin the Special Needs Aquatic membership. 

8. Comply with the SEIR Medical Rehabilitation Aquatic guidelines: 

  • Applicants must receive an Aquatic Assessment by the SEIR Medical Center for Rehabilitation aquatic staff.


  • You will also receive a Center for Rehab membership card in the mail in approximately one week from approval. You will need to scan your card at the front desk every time you use the pool or funding may be discontinued. 


  • This program is specifically for physically disabled individuals to benefit from aquatic therapy. Therefore, your greatest benefit requires regular attendance. The Special Needs Foundation Aquatic Funding recommends attendance of a minimum of (4) four Aquatic sessions per month if the pool is open. If you do not attend regularly, you may risk discontinuance of funding. Attendance will be monitored by the Aquatics Committee.


As Stated on the application process, the Special Needs Foundation Aquatic Funding requires:

  • Completed application form


  • Physician Referral form signed by their doctor stating diagnosis and health history


  • Financial Documentation showing applicant receives services from the Department of Human Services or the Social Security Administration
    • Medicaid
    • Food Assistance
    • FIP/Cash Assistance
    • Social Security
    • Disability


  • If DHS or SSA Services are not received but the applicant still qualifies, submit the most recent tax form (refer to the net income chart included with the application).
